Registration procedure

Registration to the ANRS/ISVHLD co-infections day ONLY

If you intend to ONLY register for the ANRS/ISVHLD co-infections day, you can either:

- register online
- or download the registration form, complete it and send it to the Congress Office.

Free registration for the ANRS/ISVHLD co-infections day (Wednesday July 5, 2006) is compulsory.

REGISTRATION TO ISVHLD

On-line and fax registrations are now closed. You can now register on site starting Saturday July 1, 2006 at 7:00am at the Palais des Congrès de Paris, Porte Maillot, level 2, Paris side.

Registration to ISVHLD is only open to professionals (ie: physicians, nurses, medical students and any other related medical and scientific professions).

Registration per phone will not be processed.

Registrants will receive confirmation by way of an invoice and acknowledgement letter which will be mailed and which will recall all the information given on the registration form, including the amount of Value Added Tax (VAT) paid and all other requests. This confirmation letter should be presented upon arrival at the Pre-registration Counter if the account is settled or at the on-site registration if a balance is still due.

All rates include VAT of 19,60% at current rate at the day of printing.

Please note : After June 23, 2006, pre-registration will NOT be accepted any longer by the Congress Office BUT you will have the possibility to register on site.

TERMS OF PAYMENT

Payment must be sent to the Congress Office in EUROS (€)

Copy of the bank transfer must be attached to the registration form. Do NOT forget to give the name of the person you are paying for, on the bank transfer order.

NAME CHANGES - MODIFICATIONS

A processing fee of 50€ will be charged for changes in registration except for name substitutions which will be treated as new registrations.

CANCELLATION POLICY for INDIVIDUAL REGISTRATION & RESERVATION

All cancellations must be notified in writing (by mail, fax or e.mail) to the Congress Office according to the following conditions and reimbursement will be processed AFTER the Congress:

  Up to March 20, 2006: 100 euros will be withheld for administrative fee

  Cancellations between March 21 and May 31, 2006: reimbursement of 50% of the fees paid

  After May 31, 2006: no refund.

Requests for refunds made after September 30, 2006 will not be considered.
